Welcome to our 90 day employee review process. Each employee will complete a self evaluation and their manager will complete an evaluation using the same questions. Then, you will sit down with your manager to review the results and plan for the steps ahead.
Please note, self evaluations need to be done within one week and
preferably sooner.
Employee Evaluation Scale
This is the scale you will use to rate each quality we look for in an employee.
Each question is posed in a multiple choice fashion with a mandatory answer. Each question also provides space for specific details related to your choice, but these are optional. If you review yourself above or below standard, it makes sense to add comments as to why.
Each question will have qualities listed below as a guideline to evaluation.
Significantly Exceeding Performance Goals & Expectations
High level of capability with sustained and consistent high levels of performance. Demonstrates exceptional desire and ability to succeed.
Excelling in Performance Goals & Expectations
Delivers performance frequently above job requirements. Accomplishments are often above expected levels and quality of work is uniformly higher than expected.
Fully Meeting Standards of Performance & Development
Performance clearly and fully meets all requirements of the position. Performance is solid and thorough with on time results. May display minor deviations.
Requires Additional Coaching to Achieve Expectations
Performance is noticeably less than is expected and is struggling to meet all requirements. The need for further development and improvement is recognized.
Unacceptable Performance Requiring Review Expectations
Significant area of weakness and concerns of capabilities or willingness. Performance is not meeting expectations and failing at essential job requirements.
Additional Comments & Feedback Areas Will Be Provided.
There will be an additional section at the end of the evaluation to enter comments and questions to discuss with your manager.
